Nomination Form

Please send in your nominations no later than Monday, October 23. We understand that you might not have all of the following information. Please do your best to fill out the information as completely as possible—without guessing on information.

CRITERIA FOR NOMINATIONS:

• New members to the Athletic Hall of Fame will be inducted on a biennial basis (every two years).
• In order to be eligible for nomination, all alumni must be at least ten (10) years past their graduation date in order to have eligibility.
• Coaches, support personnel and any other individuals deemed appropriate are eligible for nomination. 
• Athletes must have participated in at least two full seasons of varsity interscholastic competition at CHC. All candidates must show a record of performing with distinction and must exemplify the core values of CHC.
