Nomination Form

Nominations for the 2026 Athletic Hall of Fame open on January 1, 2025 and close on April 1, 2025. We understand that you might not have all of the following information. Please do your best to fill out the information as completely as possible—without guessing on information.

CRITERIA FOR NOMINATIONS:

• New members of the Athletic Hall of Fame will be inducted biennially (every two years).
• To be eligible for nomination, all alumni must be at least ten (10) years past their graduation date.
• Coaches, support personnel and other individuals deemed appropriate are eligible for nomination. Current coaches are eligible for nomination once they have coached at CHC for at least ten (10) years.
• Athletes must have participated in at least two full seasons of varsity interscholastic competition at CHC. All candidates must show a record of performing with distinction and exemplify the core values of CHC.
